Health systems reform in Taiwan Abstract A summary of the resource. การปฏิรูประบบสุขภาพ: กรณีศึกษาประเทศไต้หวันการศึกษานี้ทำขึ้นเพื่อนำเสนอพัฒนาการของระบบสุขภาพของไต้หวัน ทั้งในส่วนปัญหา และวิธีการแก้ไขโดยเฉพาะประเด็นการปฏิรูประบบสุขภาพ เพื่อใช้เป็นบทเรียนประกอบการพัฒนาระบบสุขภาพของไทย โดยศึกษาจากเอกสารเป็นหลักแล้วนำเสนอและวิเคราะห์ตามแนวทางการศึกษาเชิงคุณภาพ ไต้หวันประสบความสำเร็จอย่างสูงในการพัฒนาเศรษฐกิจและอุตสาหกรรม ดัชนีทางสุขภาพต่างๆ อยู่ในเกณฑ์ที่ใกล้เคียงกับประเทศที่พัฒนาแล้ว อย่างไรก็ตามการเปลี่ยนแปลงของสภาพสังคมวัฒนธรรมจากการพัฒนาประเทศ ได้ส่งผลให้เกิดปัญหาทางสุขภาพตามมา จนทำให้รัฐบาลต้องปฏิรูประบบสุขภาพของประเทศ โดยดำเนินการ 4 ด้านหลัก คือ 1) ปรับปรุงระบบเครือข่ายบริการทางการแพทย์ เพื่อการประสานการให้บริการครอบคลุมมากขึ้น 2) สร้างระบบประกันสุขภาพแห่งชาติที่ครอบคลุมประชากรทั่วประเทศ โดยรวมระบบประกันสุขภาพที่มีอยู่เดิมเข้าด้วยกันแล้วขยายให้ครอบประชากรทั้งหมด ในเดือนเมษายน 1999 ครอบคลุมประชากรได้ถึงร้อยละ 96.06 บริหารการเงินแบบ fee-for-service ร่วมกับ co-payment 3) โครงการสร้างเครือข่ายฐานข้อมูลให้สถานบริการในพื้นที่แลกเปลี่ยนข้อมูลระหว่างกันได้ และ 4) โครงการผลิตแพทย์ประจำครอบครัว ที่จะช่วยให้แพทย์ปฏิบัติงานในชุมชนและบริการฉุกเฉินได้ดีขึ้น การดำเนินงานที่ผ่านมาถือว่าประสบผลสำเร็จพอควร ปัญหาที่เป็นผลต่อเนื่องจากโครงการดังกล่าวคือประชาชนใช้บริการทางสุขภาพเพิ่มขึ้นมาก จนค่าใช้จ่ายรวมสูงกว่าเบี้ยประกันที่เก็บได้ ทำให้ต้องพยายามหามาตรการทางการเงินรวมทั้งออกกฎเกณฑ์ในการเข้ารับบริการในสถานพยาบาลให้เข้มงวดมากขึ้น ซึ่งได้กลายเป็นอุปสรรคในการเข้าถึงบริการของกลุ่มคนที่จำเป็นต้องใช้บริการด้วย บทเรียนของไต้หวันเมื่อนำมาใช้กับไทยคงต้องพิจารณาความเหมือนและความต่างของประเทศทั้ง 2 ด้วย โดยเฉพาะฐานะทางเศรษฐกิจและความเข้มแข็งทางการเมืองที่ไทยแตกต่างจากไต้หวันมาก แนวทางการปฏิรูประบบสุขภาพที่ไต้หวันดำเนินการต้องใช้งบประมาณสูงมาก ยังคงเน้นที่การปฏิรูประบบบริการสาธารณสุขเป็นหลัก และยังคงมีปัญหาด้านพฤติกรรมของประชาชนที่ก่อให้เกิดปัญหาต่อเนื่องอย่างไม่มีที่สิ้นสุด การปฏิรูประบบสุขภาพของไต้หวันอาจต้องถูกตั้งคำถามว่าจะแก้ปัญหาที่แท้จริงด้านสุขภาพได้เพียงใด ตามแนวทางนี้ธุรกิจยา เทคโนโลยีทางการแพทย์ และธุรกิจบริการสุขภาพอาจเป็นผู้ที่ได้รับผลประโยชน์มากกว่าประชาชน การเตรียมการเพื่อปฏิรูประบบสุขภาพในประเทศไทย นอกจากใช้บทเรียนจากการปฏิรูปของต่างประเทศแล้ว อาจต้องค้นหาวิธีการอื่นและมุมมองอื่นที่มาจากพื้นฐานของสังคมวัฒนธรรมไทยประกอบด้วย
