Community participation in government hospital administration Abstract A summary of the resource. การมีส่วนร่วมของประชาชนกับการบริหารโรงพยาบาลของรัฐ การศึกษาครั้งนี้ มีวัตถุประสงค์ที่จะสำรวจสถานการณ์ปัจจุบันของการมีส่วนร่วมของประชาชนในการพัฒนา/บริหารโรงพยาบาล โดยศึกษารายละเอียดของลักษณะและกระบวนการมีส่วนร่วมของประชาชน/ชุมชน ทั้งนี้โดยเก็บรวบรวมข้อมูลทั้งในเชิงปริมาณและเชิงคุณภาพ การเก็บรวบรวมข้อมูลเชิงปริมาณดำเนินโดยการส่งแบบสอบถามทางไปรษณีย์ไปยังโรงพยาบาลรัฐขนาดตั้งแต่ 60 เตียงขึ้นไป ทั้งหมดรวม 209 แห่ง และได้คัดเลือกโรงพยาบาล 5 แห่ง เพื่อทำการศึกษาเชิงคุณภาพ โดยใช้วิธีการสัมภาษณ์เชิงลึก (in-depth interviews) โรงพยาบาลที่เป็นกรณีศึกษา ได้แก่ โรงพยาบาลบ้านแพ้ว โรงพยาบาลท่าวังผา โรงพยาบาลห้วยพลู โรงพยาบาลเทียนฟ้ามูลนิธิ และโรงพยาบาลกรุงเทพคริสเตียน ผลของการสำรวจระดับกว้าง พบว่า โรงพยาบาลของรัฐแทบทั้งหมดเปิดโอกาสให้ประชาชนเข้ามามีส่วนร่วมในหลายลักษณะด้วยกัน ทั้งในด้านการบริหารจัดการโรงพยาบาล รวมทั้งกิจกรรมต่างๆ ในโรงพยาบาลและที่จัดขึ้นในชุมชน รูปแบบของการมีส่วนร่วมที่สำคัญ ได้แก่ การเป็นกรรมการที่ปรึกษา (ร้อยละ 44.2) การระดมทรัพยากรให้โรงพยาบาลโดยการบริจาคเงิน ที่ดิน การจัดหาเครื่องมือ อุปกรณ์และเวชภัณฑ์ต่างๆ ตามความขาดแคลนของโรงพยาบาล แต่แทบจะไม่มีโรงพยาบาลแห่งใดที่ให้ประชาชนเข้ามามีส่วนร่วมในการบริหาร ยกเว้นโรงพยาบาลบ้านแพ้ว แรงจูงที่สำคัญที่ให้ชุมชนเข้ามามีส่วนร่วม ได้แก่ ความต้องการให้ประชาชนสามารถเข้าถึงโรงพยาบาลมากขึ้น (ร้อยละ 89.0) และความขาดแคลนงบประมาณที่จะขยายกิจการของโรงพยาบาล (ร้อยละ 47.4) รวมทั้งการตื่นตัวจากฝ่ายประชาชนหรือชุมชนเอง (ร้อยละ 43.0) โรงพยาบาลแทบทั้งหมด (ร้อยละ 95.9) เห็นความสำคัญและประโยชน์ของการมีส่วนร่วมของประชาชน ประมาณสองในสาม (ร้อยละ 67.4) เห็นว่าการมีส่วนร่วมของชุมชนไม่ก่อให้เกิดปัญหาหรืออุปสรรคต่อการทำงาน ส่วนแนวโน้มที่โรงพยาบาลจะมีแผนปรับเปลี่ยนรูปแบบการบริหารโรงพยาบาลให้เป็นโรงพยาบาลอิสระภายใต้การกำกับของรัฐ พบว่า ประมาณหนึ่งในสี่ (ร้อยละ 26.2) มีแผนดังกล่าวเพื่อให้เกิดความคล่องตัวในการบริหารงาน ขณะที่ประมาณครึ่งหนึ่ง (ร้อยละ 51.3) ยังไม่แน่ใจ ผลการศึกษาเชิงคุณภาพได้ผลสอดคล้องกันว่า ชุมชนมีศักยภาพเพียงพอที่จะพัฒนา/บริหารโรงพยาบาลให้เป็นไปในทิศทางที่ชุมชนต้องการ การมีส่วนร่วมของชุมชน มี 4 รูปแบบใหญ่ๆ ได้แก่ 1) การมีส่วนร่วมในกระบวนการตัดสินใจด้านนโยบายและแผนพัฒนาระบบบริการ 2) การมีส่วนร่วมในกระบวนการบริหารจัดการ 3) การมีส่วนร่วมในการดำเนินกิจกรรมของโรงพยาบาล และ 4) การทำหน้าที่เป็นสื่อกลางระหว่างชุมชนและโรงพยาบาล ปัจจัยที่มีผลต่อการมีส่วนร่วมของประชาชน/ชุมชนในการบริหารจัดการโรงพยาบาล มี 3 กลุ่มปัจจัย ได้แก่ ปัจจัยด้านโรงพยาบาล ปัจจัยด้านชุมชน และปัจจัยด้านความสัมพันธ์ระหว่างชุมชนและโรงพยาบาล ปัจจัยด้านโรงพยาบาลที่สำคัญได้แก่ นโยบายการเข้าถึงชุมชน การเปิดโอกาสให้ ชุมชนเข้ามามีส่วนร่วม ศักยภาพของผู้นำหรือผู้อำนวยการ ความขาดแคลนงบประมาณจากภาคราชการ รวมทั้งการใช้กลวิธีในการระดมพลังชุมชนจากความศรัทธาต่อศาสนา ส่วนปัจจัยด้านชุมชนที่สำคัญได้แก่ ความต้องการด้านบริการสุขภาพ ศักยภาพของชุมชน ความศรัทธาในผู้นำ ความพึงพอใจในบริการ และความรู้สึกร่วมเป็นเจ้าของโรงพยาบาล รวมทั้งความสัมพันธ์ในแนวราบระหว่างโรงพยาบาลและ ชุมชน เพื่อให้โรงพยาบาลมีโครงสร้างการบริหารที่ส่งเสริมให้ประชาชน/ชุมชนมีส่วนร่วมในการบริหารของโรงพยาบาล จำเป็นที่จะต้องมีการปรับเปลี่ยนโครงสร้างการบริหารโดยจัดให้มีคณะกรรมการบริหารที่มาจากฝ่ายประชาชนทั้งหมดหรือบางส่วน ไม่ต่ำกว่าครึ่งหนึ่งของกรรมการบริหาร ทั้งหมด ทั้งนี้เพื่อให้ชุมชนมีอำนาจตัดสินใจเชิงนโยบายและแผน รวมทั้งการบริหารจัดการการดำเนินกิจกรรมต่างๆของโรงพยาบาล ตลอดจนการควบคุมกำกับและตรวจสอบการบริหารงานของ โรงพยาบาลโดยแท้จริง เพื่อให้เกิดการมีส่วนร่วมของชุมชนที่ยั่งยืน รัฐและโรงพยาบาลจำเป็นต้องสร้างเงื่อนไขเกี่ยวกับสภาพแวดล้อมทางสังคมและเศรษฐกิจชุมชนให้เอื้ออำนวยต่อการมีส่วนร่วม เงื่อนไขที่จำเป็นได้แก่ การสร้างกระแสความตื่นตัวของชุมชนในระบอบประชาธิปไตย การสนับสนุนให้เกิดการรวมกลุ่มในลักษณะประชาสังคม การปลุกกระแสให้เกิดความรู้สึกร่วมเป็นเจ้าของโรงพยาบาลและรู้จักรักษาสิทธิ ผู้ป่วย นอกจากนั้น โรงพยาบาลจะต้องให้ความสำคัญกับนโยบายเข้าถึงชุมชนโดยจัดบริการเชิงรุกเกี่ยวกับการส่งเสริมสุขภาพของประชาชน ตลอดจนจัดให้มีโครการพัฒนาสุขภาพผสมผสานไปกับโครงการพัฒนาเศรษฐกิจของชุมชน เพื่อสร้างชุมชนให้เข้มแข็งและมีศักยภาพทางเศรษฐกิจเพียงพอที่จะเอื้อ ต่อการมีส่วนร่วมของชุมชนในการพัฒนา/บริหารโรงพยาบาล ทั้งนี้โดยการประสานความร่วมมือกับ องค์กรชุมชนในท้องถิ่น ทั้งองค์กรของรัฐและเอกชนรวมทั้งวัดซึ่งเป็นศูนย์กลางของชุมชน ในการกำหนดกฎระเบียบที่เกี่ยวข้องกับการมีส่วนร่วมของชุมชน อาทิเช่น องค์ประกอบของคณะกรรมการบริหารโรงพยาบาล คุณสมบัติของผู้แทนชุมชนที่จะเข้ามาเป็นคณะกรรมการบริหาร โรงพยาบาล รวมทั้งขั้นตอนของการได้มาซึ่งผู้แทนชุมชน ฯลฯ ควรจะให้ชุมชนมีส่วนร่วมในกระบวนการพิจารณาตัดสินใจ ทั้งนี้เพื่อให้สอดคล้องกับความต้องการและสอดคล้องกับสภาพสังคมวัฒนธรรมของชุมชน ตลอดจนมีความเป็นไปได้ในทางปฏิบัติ http://164.115.27.97/digital2024/files/original/ae71bab43dfff824f465d585abd140f5.pdf 3a2ebbb64c959b9bb222acdc3ccc532b Dublin Core The Dublin Core metadata element set is common to all Omeka records, including items, files, and collections.
