Process and outcomes study on rehabilitation service for hemiplegic patient causing by stroke at the Sirindhon National Medical Rehabitation Center Abstract A summary of the resource. การศึกษากระบวนการและผลลัพธ์การบริการฟื้นฟูสมรรถภาพผู้ป่วยอัมพาตจากโรคหลอดเลือดสมอง แบบผู้ป่วยในของศูนย์สิรินธรเพื่อการฟื้นฟูสมรรถภาพทางการแพทย์แห่งชาติ วัตถุประสงค์งานวิจัยนี้คือ เพื่อศึกษากระบวนการและผลลัพธ์ การบริการฟื้นฟูสมรรถภาพแบบผู้ป่วยใน สำหรับผู้ป่วยอัมพาตจากโรคหลอดเลือดสมอง ของศูนย์สิรินธรเพื่อการฟื้นฟูสมรรถภาพทางการแพทย์แห่งชาติ โดยเป็นการศึกษาแบบย้อนหลังจากเวชระเบียนผู้ป่วยใน ระหว่างปีงบประมาณ 2538-2542 ร่วมกับการศึกษาเชิงคุณภาพ โดยเก็บข้อมูลภาคตัดขวางของผู้ป่วยที่รับบริการและกลับบ้านได้ในปีงบฯ 2542 จากการศึกษาเวชระเบียน พบว่ามีการรับผู้ป่วยอัมพาตจากโรคหลอดเลือดสมอง เข้าฟื้นฟูฯแบบผู้ป่วยใน ในเวลาห้าปี รวม 166 ครั้ง (137 คน) คิดเป็นประมาณร้อยละ 25 ของการรับเข้าอยู่ทั้งหมด ซึ่งเป็นการรับเข้าอยู่ที่อยู่ในเกณฑ์เฉลี่ยที่กำหนดเพียง 111 ครั้ง (111 คน) คิดเป็นร้อยละ 67ผู้ป่วยที่รับเข้าอยู่ 111 ครั้ง เป็นชาย:หญิง ประมาณ 3:2 อายุเฉลี่ย 60.67+11.96 ปี (95%CI for mean = 58.42-62.92 ปี) เป็นอัมพาตซีกซ้ายมากกว่าซีกขวาประมาณ 2-3 เท่า สาเหตุของการเป็นอัมพาตสามารถระบุได้ชัดเจนเพียงร้อยละ 75 ในจำนวนนี้ เป็นจาก Intracerebral infarction มากกว่า Intracerebral haemorrhage ประมาณ 2 เท่า ผู้ป่วยส่วนใหญ่ไม่เสียค่าบริการเอง เนื่องจากมีรัฐสวัสดิการประเภทต่างๆ และส่วนน้อยที่ต้องจ่ายเองในตอนแรกรับ เมื่ออยู่ไประยะหนึ่งก็ขอรับสิทธิ์ฟรีตามพรบ.ฟื้นฟูสมรรถภาพคนพิการ พ.ศ.2534การรับเข้าอยู่เพื่อฟื้นฟูฯผู้ป่วยกลุ่มนี้ มีจำนวนวันนอน/รายเฉลี่ย เป็น 44.22+28.36 วัน (95%CI for mean = 38.88-49.55 วัน) ทั้งนี้มีจุดตัดวันนอนที่สั้นหรือนานเกินที่ 7 และ 150 วัน เมื่อปรับจุดตัดวันนอนเป็น 13 และ 84 วัน ทำให้วันนอน/รายเฉลี่ย เป็น 42.31+19.11 วัน (95%CI for mean = 38.18-46.43 วัน)ผลลัพธ์ของการฟื้นฟูฯผู้ป่วยอัมพาตฯที่รับบริการในปี 2542 พบว่าได้ข้อมูลจากแบบสอบถาม 13 ราย จากทั้งหมดที่รับเข้าฟื้นฟูฯ 48 ราย ซึ่งความสามารถในการทำกิจวัตรประจำวัน (Activity of daily living, ADL.) เป็นดังนี้ 10/13 ราย สามารถควบคุมการขับถ่ายได้เป็นปกติ 6/13 ราย กินอาหาร/ล้างหน้า/แปรงฟัน ได้เอง 4/13 ราย สามารถทาแป้ง/หวีผม/โกนหนวด/ใช้ห้องส้วม/ใส่-ถอดเสื้อผ้า ได้เอง มีเพียง 1/13 รายเท่านั้นที่อาบน้ำเองได้ ส่วนการเดิน (Ambulation) พบว่า 7/13 ราย สามารถเดินได้เองบนพื้นราบ 5/13 ราย สามารถเคลื่อนย้ายตัวเอง (Transfer) ได้ดี สำหรับความสามารถในการดำรงชีวิต สามารถสื่อสารได้ดี 9/13 ราย ถูพื้นได้ 3/13 ราย ออกนอกบ้านได้เอง 2/13 ราย เตรียมอาหารได้เอง 1/13 ราย คนที่ทำไม่ได้หมายถึง ต้องมีคนคอยช่วยเหลือดูแล นั่นคือผู้ป่วยที่ได้รับการฟื้นฟูฯ สามารถช่วยตัวเองได้ระดับหนึ่ง ซึ่งยังบอกไม่ได้ว่านี่คือผลของกระบวนการฟื้นฟูฯทั้งหมด อย่างไรก็ตามก็ยังไม่เพียงพอที่จะทำให้ผู้ป่วยกลุ่มนี้สามารถดำรงชีวิต อย่างน้อยภายในบ้านด้วยตนเองได้ทั้งหมด นอกจากนั้นการศึกษารายกรณีอีก 4 ราย ยังสะท้อนให้เห็นว่าผู้ป่วยแต่ละรายยังมิได้หยุดการแสวงหาการรักษาและฟื้นฟูฯภาวะอัมพาตฯ และระบบการติดตามดูแล เพื่อให้รู้ปัญหาเพื่อรักษาระดับผลการฟื้นฟูฯที่ได้ และสนับสนุนการปรับตัวให้ดำรงชีวิตได้ อย่างมีคุณภาพ ยังไม่เพียงพอ http://164.115.27.97/digital2024/files/original/5c675888ad0c1e610d4417f9b1d3ad7c.pdf 206d00e98866ef0298585f9871e51378 Dublin Core The Dublin Core metadata element set is common to all Omeka records, including items, files, and collections. Draft report on financing universal health care in Thailand Abstract A summary of the resource. โครงการหลักประกันสุขภาพถ้วนหน้าเป็นนโยบายที่สำคัญของรัฐบาล ครอบคลุมประชากรไทยร้อยละ 75 อยู่ แต่ได้รับการงบประมาณอุดหนุน ด้วยอัตราเหมาจ่ายรายหัวที่จำกัดต่ำกว่าข้อเท็จจริงของอัตราการใช้ และต้นทุนบริการ และมีปัญหาความยั่งยืนทางการเงินระยะยาวของโครงการ การศึกษานี้วิเคราะห์และเสนอแนวทางการปฏิรูปการเงินการคลังของโครงการฯ โดยการประมาณการรายจ่ายสุขภาพระยะยาวของประเทศไทยระหว่าง ปี 2547-2563 โดยใช้แบบจำลอง (model) สี่แบบ ได้แก่ แบบจำลองด้านประชากรและแรงงาน แบบจำลองด้านเศรษฐกิจ แบบจำลองค่าใช้จ่ายด้านสุขภาพ และแบบจำลองงบประมาณของรัฐบาล โดยใช้ฐานข้อมูลประชากรและเศรษฐกิจ ของสำนักงานคณะกรรมการพัฒนาการเศรษฐกิจและสังคมแห่งชาติ สถิติแรงงาน ของสำนักงานสถิติแห่งชาติ บัญชีรายจ่ายสุขภาพแห่งชาติของสำนักงานพัฒนานโยบายสุขภาพระหว่างประเทศ ข้อมูลงบประมาณประจำปี จากกรมบัญชีกลาง กระทรวงการคลัง นอกจากนี้ อาศัยสมมติฐานต่างๆที่คาดว่าน่าจะเป็น พร้อมทั้งการสร้างฉากทัศน์ทั้งการลดรายจ่าย และ/หรือ การเพิ่มงบประมาณที่แน่นอนของโครงการฯ ผลการศึกษา พบว่า ปัจจุบันรายจ่ายสุขภาพทั้งประเทศ เท่ากับร้อยละ 3.50 ของผลิตภัณฑ์มวลรวมภายในประเทศ ในปี 2547 จะเพิ่มขึ้นเป็นร้อยละ 3.64 และ 3.88 ในปี 2553 และ 2563 ตามลำดับ ฉากทัศน์ที่ 1 กำหนดให้โครงการฯ มีรายได้เฉพาะที่แน่นอนจากการเก็บภาษีสุขภาพ (Personal health tax) การปรับเพิ่มภาษีบุหรี่ สุรา และเบียร์แล้วกันเงินภาษีส่วนหนึ่งให้แก่โครงการฯเป็นประจำ (Earmarked sin tax) โครงการฯจะมีความต้องการเงินอุดหนุนจากงบประมาณประจำปีของรัฐลดลงอย่างมากเมื่อเทียบกับสภาพปัจจุบัน ฉากทัศน์ที่ 2 กำหนดให้จำนวนผู้มีสิทธิ์ในโครงการฯ ลดลง โดยการขยายความคุ้มครองประกันสังคมไปสู่คู่สมรสและบุตรของผู้ประกันตน ทำให้รายจ่ายของโครงการฯ ลดลง 9,210 ล้านบาทในปี 2548 ฉากทัศน์ที่ 3 เป็นควบรวมฉากทัศน์ที่ 1 และ 2 ทำให้มีการลดรายจ่าย (โดยการขยายการคุ้มครองของประกันสังคม) พร้อมกับการเพิ่มรายรับที่แน่นอนเข้ามาในโครงการ ทำให้โครงการฯ เกือบไม่มีความต้องการเงินอุดหนุนจากงบประมาณประจำปีเลย ข้อเสนอเชิงนโยบายต่อการปฏิรูปการเงินการคลังโครงการหลักประกันสุขภาพถ้วนหน้าให้มีความยั่งยืน และมีความเป็นไปได้ มี 3 มาตรการ มาตรการที่หนึ่ง การตัดโอนภาษีสรรพสามิตที่รัฐจัดเก็บได้มาให้โครงการฯเป็นการประจำตามจำนวนที่กำหนดไว้ (Earmarked sin tax) โดยไม่ต้องปรับเพิ่มภาษี หรืออาจจะปรับเพิ่มภาษี ซึ่งทำให้สามารถจัดตั้งกองทุนหลักประกันสุขภาพแห่งชาติได้ มาตรการที่สอง การลดรายจ่ายของกองทุนฯโดยให้ระบบประกันสังคมขยายการคุ้มครองไปสู่คู่สมรสและบุตรของผู้ประกันตน ซึ่งอาจกระทำได้โดยไม่ต้องปรับเพิ่มอัตราเงินสมทบจากผู้ประกันตน นายจ้าง และรัฐบาล มาตรการที่สาม ปฏิรูปพระราชบัญญัติคุ้มครองผู้ประสบภัยจากรถ โดยมอบให้กรมการขนส่งทางบก กระทรวงคมนาคม เป็นผู้จัดเก็บเบี้ยประกันภัย พร้อมกับการเก็บภาษีประจำปีจากเจ้าของรถตามกฎหมาย หรือขณะทำการจดทะเบียนรถใหม่ ทำให้สามารถบังคับใช้กฎหมายได้เต็มที่ โดยผู้ประสบภัยจากรถทุกรายไม่ว่าเกิดจากรถ ที่มีหรือไม่ พ.ร.บ. การใช้เงินของ พรบ. นี้จะมีประสิทธิภาพมากขึ้น
