Traffic victim protection act in the contest of universal coverage : What are the reform option Abstract A summary of the resource. พ.ร.บ. คุ้มครองผู้ประสบภัยจากรถดำเนินการมาตั้งแต่ปี 2535 แม้ว่าจะเป็นการประกันภาคบังคับ บริษัทประกันวินาศภัย ที่มุ่งกำไร เป็นผู้บริหารโครงการนี้ทั้งหมด แทนที่จะเป็นภาครัฐ พ.ร.บ.นี้ ให้การคุ้มครองโดยจ่ายสินไหมทดแทนกรณีรักษาพยาบาล ไม่เกินเพดานที่กำหนด และเหมาจ่ายกรณีการตาย และทุพพลภาพ รายงานปี 2545 แสดงให้เห็นว่า สามารถเก็บเบี้ยประกันได้ 7,003 ล้านบาท โดยมีรายจ่ายเพื่อการบริหารสูงถึงร้อยละ 41 รายจ่ายสินไหมทดแทนร้อยละ 52 พ.ร.บ. ไม่สามารถคุ้มครองผู้ประสบภัยจากรถทุกคนได้ เมื่อประเทศไทยบรรลุประกันสุขภาพถ้วนหน้าแล้ว มีความจำเป็นต้องทบทวน ความเหมาะสมและเสนอแนวทางการปฏิรูป ให้เหมาะสม การศึกษานี้มีวัตถุประสงค์เพื่อประมาณการความต้องการงบประมาณเพื่อคุ้มครองผู้ประสบภัยจากรถทุกคน และประมาณการว่าหากจะเก็บภาษีน้ำมันเพิ่มขึ้น จะต้องเก็บอีกกี่สตางค์ต่อลิตร เพื่อให้ได้เงินเพียงพอในการคุ้มครองผู้ประสบภัยจากรถทุกคน และนำเสนอแนวทางการปฏิรูป การศึกษาพบว่า เพื่อให้การคุ้มครองผู้ประสบภัยทุกคน ต้องการงบประมาณ 7,158 ล้านบาท ในปี 2545 เป็นการรักษาพยาบาลกรณีผู้ป่วยในร้อยละ 63 การรักษาก่อนถึงโรงพยาบาลร้อยละ 16 กรณีตายร้อยละ 15 กรณีรักษาพยาบาลผู้ป่วยนอกร้อยละ 5 และกรณีทุพพลภาพร้อยละ 1 อาศัยฐานข้อมูลการสำรวจภาวะเศรษฐกิจและสังคมของครัวเรือน ปี 2545 โดยสำนักงานสถิติแห่งชาติ ผู้วิจัยประมาณรายจ่ายบริโภคน้ำมันของครัวเรือน และปรับรายจ่ายเป็นปริมาณการบริโภค (ลิตร) จำแนกตามเดไซล์ของรายได้ครัวเรือน เพื่อวิเคราะห์ภาระรายจ่ายครัวเรือนหากจะต้องจ่ายภาษีน้ำมันเพิ่มขึ้น ผู้วิจัยพบว่า จะต้องเก็บภาษีเพิ่มขึ้นลิตรละ 32 สตางค์ เพื่อให้ได้เงินทั้งสิ้น 7,158 ล้านบาท เพื่อการคุ้มครองผู้ประสบภัยจากรถทุกคน หากไม่มีความยืดหยุ่นของราคาต่อปริมาณและแบบแผนของอุปสงค์ ผู้วิจัยเสนอว่าให้เก็บภาษีเบนซิน 95 สูงกว่า น้ำมันประเภทอื่น ทางเลือกที่เก็บเพิ่มอีกลิตรละ 26 สตางค์สำหรับน้ำมันประเภทอื่นๆ และ 55 สตางค์สำหรับเบนซิน 95 เป็นทางเลือกที่มีความถดถอยน้อยที่สุดและมีความเป็นไปได้และการยอมรับมากกว่า การปฏิรูปควรจะบรรลุเป้าหมายหลัก 2 ประการคือ ให้การคุ้มครองทุกคน และต้นทุนในการบริหารจัดการต่ำ ผู้วิจัยเสนอ 3 ทางเลือกจำแนกตามที่มาของเงิน ทางเลือกที่ 1 อาศัยเบี้ยประกันตาม พ.ร.บ. ทางเลือกที่ 2 อาศัยภาษีทั่วไป และทางเลือกที่ 3 อาศัยภาษีน้ำมันเฉพาะ ผลการวิเคราะห์พบว่า ทางเลือกที่ 2 และ 3 มีความเป็นไปได้น้อย เนื่องจาก มีเบี้ยประกันอยู่แล้ว ไม่น่าจะเป็นภาระงบประมาณ และในภาวะวิกฤตน้ำมัน ทางเลือกที่ 3 ไม่น่าจะเป็นที่ยอมรับได้ ภายใต้ ทางเลือกที่ 1 ผู้วิจัยเสนอว่า การปฏิรูปใหญ่ (major reform) โดยให้กรมขนส่งทางบกเก็บเบี้ยประกันในขณะเดียวกับการชำระภาษีรถยนต์ประจำปี โดยจัดงบประมาณบางส่วนจากเบี้ยประกันฯ เพื่อการจัดเก็บอย่างเหมาะสม เบื้ยที่เหลือส่งให้สำนักงานหลักประกันสุขภาพแห่งชาติ (สปสช.) เป็นผู้ดำเนินการจ่ายกรณีเจ็บป่วย ตาย และทุพพลภาพ ทางเลือกการปฏิรูปเล็กน้อย (minor reform) ก็อาจจะเป็นที่ยอมรับได้ เนื่องจากสามารถบรรลุวัตถุประสงค์ทั้ง 2 ด้านคือต้นทุนบริหารจัดการต่ำ และให้การคุ้มครองผู้ประสบภัยทุกคน http://164.115.27.97/digital2024/files/original/f92e8c31b398a2265601fa542132e66c.pdf 3333a1cbd3b1bd27d35a5aa9f9f47507 Dublin Core The Dublin Core metadata element set is common to all Omeka records, including items, files, and collections.
