Care-taker behavior and glycemic control of elderly non-insulin dependent diabetic patients, Amphur Khaowong, Kalasin Province Abstract A summary of the resource. การวิจัยนี้เป็นการวิจัยเชิงสำรวจ (Survey Research) โดยใช้ข้อมูลในช่วงเวลาที่กำหนด (Cross-Sectional study) ซึ่งมีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างปัจจัยส่วนบุคคลและพฤติกรรมของผู้ดูแลกับภาวะการควบคุมโรคในผู้ป่วยเบาหวานสูงอายุชนิดไม่พึ่งอินซูลิน อำเภอเขาวง จังหวัดกาฬสินธุ์ จำนวนผู้ดูแลและผู้ป่วยกลุ่มละ 127 คน เก็บข้อมูลทั้งในเชิงปริมาณและเชิงคุณภาพตั้งแต่เดือนพฤศจิกายน 2541 ถึงกุมภาพันธ์ 2542 จากนั้นนำผลที่ได้มาวิเคราะห์ข้อมูลเชิงปริมาณโดยใช้สถิติเชิงพรรณนา, ค่าไคสแควร์, odd ratio ที่ความเชื่อมั่น 95% และ multiple logistre regression ส่วนข้อมูลเชิงคุณภาพใช้การวิเคราะห์เชิงเนื้อหา (content analysis) ผลการวิจัยสรุปได้ดังนี้ ปัจจัยส่วนบุคคลและพฤติกรรมของผู้ดูแลที่มีผลทำให้ภาวะการควบคุมโรคของผู้ป่วยเบาหวานสูงอายุแตกต่างกันอย่างมีนัยสำคัญทางสถิติเมื่อมีการควบคุมปัจจัยอื่น ได้แก่ อายุ ผู้ดูแลที่มีอายุ 60 ปีขึ้นไปจะมีผลทำให้ผู้ป่วยเบาหวานสูงอายุควบคุมระดับน้ำตาลในเลือดได้ดีกว่าผู้ดูแลที่มีอายุระหว่าง 30-59 ปี 18 เท่า (95% CI = 1.4-136.3; p-value = 0.02)สถานภาพสมรส ผู้ดูแลที่เป็นโสดจะมีผลทำให้ผู้ป่วยเบาหวานสูงอายุควบคุมระดับน้ำตาลในเลือดได้ดีกว่าผู้ดูแลที่สมรสแล้ว 1.9 เท่า (95% CI = 1.2-94.9; p-value = 0.03)รายได้ครอบครัว ผู้ดูแลที่มีรายได้ครอบครัวเฉลี่ยต่อปีต่ำกว่า 10,000 บาท จะมีผลทำให้ผู้ป่วยเบาหวานสูงอายุควบคุมระดับน้ำตาลในเลือดได้ดีกว่าผู้ดูแลที่มีรายได้ครอบครัวเฉลี่ยต่อปี 10,000 ถึง 90,000 บาท 7.4 เท่า (95% CI = 1.6-36.3; p-value = 0.01)เวลาที่อยู่กับผู้ป่วยในแต่ละวัน ผู้ดูแลที่มีเวลาอยู่กับผู้ป่วยมากกว่า 12 ชั่วโมงต่อวันจะมีผลทำให้ผู้ป่วยเบาหวานสูงอายุควบคุมระดับน้ำตาลในเลือดได้ดีกว่าผู้ดูแลที่มีเวลาอยู่กับผู้ป่วยน้อยกว่าหรือเท่ากับ 12 ชั่วโมงต่อวัน 9.6 เท่า (95% CI = 1.7-55.1; p-value = 0.01)ผู้ดูแลที่มีคะแนนทัศนคติสูงกว่าหรือเท่ากับค่าเฉลี่ยจะมีผลทำให้ผู้ป่วยเบาหวานสูงอายุควบคุมระดับน้ำตาลในเลือดได้ดีกว่าผู้ดูแลที่มีคะแนนทัศนคติต่ำกว่าค่าเฉลี่ย 4.8 เท่า (95% CI = 1.2-19.9; p-value = 0.02) และจากการสัมภาษณ์เชิงลึกพบว่ากลุ่มผู้ดูแลผู้ป่วยเบาหวานสูงอายุที่ควบคุมระดับน้ำตาลในเลือดได้ดีและกลุ่มผู้ดูแลผู้ป่วยเบาหวานสูงอายุที่ควบคุมระดับน้ำตาลในเลือดได้ไม่ดีมีความรู้ในการควบคุมโรคเบาหวานไม่มากนัก โดยความรู้ที่ได้รับส่วนใหญ่จะมาจากตัวผู้ป่วยและเพื่อนบ้านที่เป็นเบาหวาน แต่อย่างไรก็ดีจะพบว่าผู้ดูแลผู้ป่วยเบาหวานที่ควบคุมระดับน้ำตาลในเลือดได้ดีจะให้การดูแลในเรื่องเกี่ยวกับการควบคุมอาหาร, การออกกำลังกาย และการมาตรวจตามนัดได้ดีกว่าผู้ดูแลผู้ป่วยเบาหวานที่ควบคุมระดับน้ำตาลในเลือดได้ไม่ดี
